Solution:
Count the points after the first 3 tricks. You should know by now that declarer (North) has shown the Q and 
J, and implicitly has possession of the 
A and A. That is 11 points. And he opened 1NT (15-17). Granted, some open 1NT with 14 with a good 5-card suit or/and plenty intermediate 9's and 10's, but here declarer is 98% sure to possess the HA instead of the CK to make up his quota. Furthermore, if partner has the A, you cannot beat this contract as he will have at most three hearts.
Switch immediately to the 
A - Q - 4 then!
